We ask your consideration and request that if you must cancel an appointment you notify us at least 24 hours in advance. There will be a charge for broken appointments without notice.
We will accept assignment and process forms for those patients with dental insurance providing:
1. Patients provide us with a completed, signed dental insurance form at the time of each visit (unless your dental insurance carrier is Delta or BCIBS who do not require your forms).
2. All financial agreements are with the patient - your contract with your insurance carrier is your contract, not ours.
3. We expect co-payment to be made at the time of each visit of the estimated share of the costs that your insurance will not cover.
For patients without dental insurance:
We expect payment at the time services are provided, unless there is a specific financial arrangement in place.
For your convenience, we do accept VISA, MasterCard, Discover Card, and CARE CREDIT.
All patients requiring major work must have a written financial arrangement, which will require a minimum of one-third (1/3) down payment before the case is started and payment in full when treatment is completed.
